2. How do I know if I am eligible for a rebate?
If there's an active rebate offer for your purchased product and purchase date, it will be on the Alltel Rebate Center website.
3. What do I need to submit a rebate claim?
You will need the applicable rebate form, sales receipt or packing slip and the proof of purchase label including the UPC barcode from the device box. PLEASE READ ALL OF THE VALIDATION RULES OUTLINED ON THE REBATE FORM CLOSELY FOR EACH OFFER.
4. Where is the UPC barcode located?
The UPC is the 12 digit code located on the product box. The original barcode must be submitted for the rebate to be processed. Copies will not be accepted.
5. I threw away my product box. Can I still submit a rebate claim?
Unfortunately, you must have the original label to be eligible for a rebate.
6. Why do I need to make a copy of my rebate documentation before I submit my rebate?
Once the rebate documentation has been submitted, it will not be returned to you. It may be helpful to have copies of the documentation in the event you have questions regarding your rebate claim.
7. What name and wireless phone number do I put on the rebate form?
Please provide the name on your Alltel account as well as the wireless phone number associated with the handset purchase and 2 year contract.
8. When can I expect to receive my rebate?
You may check the status of you rebate claim at the Alltel Rebate Center website.
9. If I purchased more than one wireless device that is eligible for a rebate, can I mail more than one rebate request within the same envelope?
Each rebate request MUST be submitted separately and mailed in individual envelopes.
10. What is the deadline for submitting the rebate?
Please refer to the rebate form for specific time periods, including valid offer dates and postmark dates along with promotional requirements.
11. How do I resubmit to validate my rebate claim if I was denied for not providing a piece of required documentation? i.e. Receipt, Original UPC, etc?
Resubmissions must be submitted within 30 days of being notified of invalid status. There are two options for resubmissions:
1) You may fax ALL required documentation to validate the claim along with the assigned submission tracking number to (866) 964-7065. (Please print out a confirmation page verifying that the faxed information did go through successfully.)
2) You may mail in ALL required documentation to validate the claim along with the assigned submission tracking number to:
Alltel Resubmissions Dept.If you MAIL IN your resubmission, please allow 4 - 6 weeks from your resubmission date for reprocessing of your rebate. If you FAX your resubmission, please allow 2 - 3 business days from your resubmission date for reprocessing of your rebate. After the allotted time, please check the status of your rebate at www.alltelrebatecenter.com.

12. How do I check the status of my rebate?
Click the button titled “TRACK YOUR REBATE” at the top of the page. Enter your tracking number, MDN number, or name and zip code. If you are unable to locate your record, please contact a rebate customer service representative at 1-800-460-0942.
